RESERVATION ACCURACY
By submitting this form I attest that the information submitted is accurate and agree to be held to Integrity World Travel/Cruzdiva.com terms and conditions (available on our website) and authorize said company to charge the stated amount above to my credit card provided herein. I further agree to pay for this purchase in accordance with the issuing bank cardholder agreement.
Due to the complexity of servicing reservations our agency charges a $50.00 per person non-refundable service fee in addition to the cost of your reservation should you need to cancel or modify your reservation.
FINANCIALS
I fully understand that my deposit is non-refundable and non-transferable and AFTER final payment my entire reservation is non-refundable. I also understand there a $50.00 per person administrative cancellation fee in addition to the respective supplier/hotel/airline/tour operator/transportation company's cancellation fees. If my reservation/package includes air travel, I understand my airline tickets are NON-REFUNDABLE and agree to the United States TSA regulations.
If my reservation includes airline tickets and I have paid a reduced deposit [a deposit ranging from $250-$500 per person] I understand I will forfeit the entire deposit/airline credit in the event I cancel. Additionally, if I have submitted a reduced deposit, I understand any changes will either 1. forfeit the entire deposit or 2. require I pay the full amount of the airline ticket BEFORE any changes can be made to my reservation.
3. Shared Financial Responsibility Reservations (Roommates)
All rooms/staterooms must be paid in full at minimum double occupancy prior to departure. If the reservation’s financial responsibility is shared by more than one guest, each responsible party must be fully paid prior to sailing. Should an individual(s) financial responsibility not be fulfilled, the other individual(s) on the reservation will be responsible for the remaining funds.
Cruzdiva.com will not be responsible or liable for any issues related to roommate agreements. Any problems associated with cabin/roommates (both on board or during the reservation and payment process) will be subject to the set forth terms and conditions and the other passengers on the reservation will be held responsible for full payment.
4. TRAVEL INSURANCE
Should I choose to protect my travel investment by purchasing travel insurance I understand this is an auxiliary service and is provided as a courtesy, is a separate entity not associated directly with Integrity World Travel/ Cruzdiva.com. I understand processing of insurance claims and benefits are at the sole discretion of the respective insurance company and agree to hold Integrity World Travel/ Cruzdiva.com harmless from any issues arising with travel insurance.
5. TRAVEL DOCUMENTATION
I understand international travel (other than cruises that originate and return to a United States port) require a valid passport book for passengers of all ages and that certain countries require that my passport be valid for a minimum of 6 months beyond my return date. I understand failure to present required document will result in being denied travel. Integrity World Travel/Cruzdiva.com will not be held liable for client's failure to present required documentation or denial of travel.
